Beyond Stem Cells: What You Should Know About RPA Therapy
If you’ve dealt with a stubborn shoulder, back, or joint problem, you’ve probably already been through the standard playbook. Over-the-counter pain relievers. A stretch of physical therapy. Maybe a corticosteroid shot. For a lot of patients, that helps for a while, and then it doesn’t, or it comes with tradeoffs nobody’s thrilled about. That’s a big part of why regenerative medicine keeps growing as a field, and why we’ve added a new option here at Nivens Spine Medicine: Regenerative Protein Array (RPA) and MUSE-Infused RPA.
Not as new as it sounds
Regenerative medicine gets talked about like it appeared last year, but the underlying idea, helping the body heal itself, goes back a lot further than that. It’s the product of decades of tissue engineering and surgical advances building on each other. RPA is just the latest version of that idea: using biologic material to support the body’s repair process instead of replacing tissue outright.
Where the usual options run into limits
NSAIDs are easy to get and usually tolerated fine, but long-term use isn’t free of risk, and they don’t touch the underlying problem. Physical therapy is genuinely one of the better long-term tools for pain and function, though sticking with it is hard when the program itself is uncomfortable or eats up your schedule. Corticosteroid shots can knock down pain and swelling quickly, but a lot of patients notice each one works a little less than the last. None of that makes these options bad, they’re still a real part of a lot of care plans, but it explains some of the interest in alternatives.
Ten years of watching this space
Dr. Nivens has been following regenerative medicine for about a decade, and he’s the first to say he didn’t get behind it right away. “Initially there was a lot of misinformation out there, but also not a lot of information,” he says of when traditional stem cell products first showed up. “I never felt comfortable about getting behind this product, mainly because there was not a lot of research. People were just injecting it into folks and hoping that it would do what they would hope it would do.” Traditional stem cell therapy also comes with a real tradeoff: patients receive live cellular material and DNA from a donor, and that’s been tied to rejection issues and, in some cases, unwanted tissue growth.
What changed his mind
What shifted things for Dr. Nivens was a company called Genesis Regenerative Medicine, which found a way to filter that same donor material so patients get the proteins, exosomes, and other cell products without any of the cellular material or DNA that comes with it. “You’re strictly getting the products within the cells,” he says, “and you’re not getting any crossover of cellular material from the host where they harvest these cells.” That filtered product is what’s now offered as RPA and MUSE-Infused RPA: an array of proteins, growth factors, and cytokines drawn from donated placental tissue, verified by third-party proteomic and PCR testing to contain more than 300 healing proteins from all three layers of the placenta. MUSE-Infused RPA adds a concentrated dose of naturally stress-resistant proteins for patients who could use the extra support.
“I’ve been this way about any new drug that’s on the market,” Dr. Nivens says. “I always wait to see how this drug is going to perform before offering it to my patients. There is actual science and data now that we can turn to and rely on, that I will feel comfortable giving to my patients. It’s not just a product that doctors are injecting into anyone.”
How it works, in his words
Asked what these products actually do, Dr. Nivens keeps it simple: “They really focus on repairing soft tissue, connective tissue, ligaments, tendons, discs, and they’re very good at supporting our body’s natural function of repairing. These can enhance that.” He points to inflammation as the other piece. “They work nicely to reduce inflammatory processes, and that is the basis of our pain,” he says. “That typically occurs because of damaged cells or damaged tissues. These products have an affinity to really go in and try to work with the existing mechanism of healing, and enhance that.”
Where it fits in your care
RPA isn’t a replacement for the musculoskeletal and spinal care Dr. Nivens already provides, it’s another option within it. “If we’ve done the steroid injections, or this is a surgical indication because you’re not responding to conservative treatment, this is another avenue to pursue,” he says. “We might be able to mitigate some of the pain that you’re having.” He’s careful not to oversell it: “These products, again, are not a panacea for everything, but they are showing a strong potential to help the healing process.”
How it’s sourced and checked
Donors are U.S.-based and come from scheduled, healthy C-section births, screened for a 20-year history clear of cancer, autoimmune disease, and genetic disease, and tested for communicable disease. The product then goes through quad-filtering to remove cellular and DNA matter, plus third-party endotoxin and 14-day sterility testing. It’s produced in an FDA-registered facility under Human Cellular Therapy/Tissue Product standards. Results vary from person to person, but the testing is built to keep safety front and center.
Is it worth asking about?
It won’t be the right fit for every patient or every condition.
